There indeed was a little bug in code. Took some time to find it, but it's now fixed and there shouldn't be any more disappearing tasks. :)
Seems like there's a bug somewhere in there. We'll investigate and get it fixed.
I have had a similar issue to this with the production "Saint Dymphna", tasks appear in the site wide active tasks list, but not in the task list for Saint Dymphna.
Maybe it's classified.
